- CDS Coordinator
The Consumer Directed Services (CDS) Coordinator assists persons with physical disabilities in accessing consumer-directed personal attendant services and in achieving goals they have set for themselves to decrease dependence on others. The CDS Coordinator provides case management services, skills training and information and referral services, and performs record-keeping functions. The CDS Coordinator advocates for the needs of persons with disabilities and documents services provided. In addition, the CDS Coordinator is responsible for: Providing the Four-Core Services of Independent Living; Provide one-to-one or group training in skills necessary for consumers on this program to be effective employers of their attendants. Such training includes record-keeping, employer tax obligations, and how to hire, train and manage personal attendants; Advocate for the collective needs of people with disabilities; monitor activities in the community for potential discrimination; assist consumers in filing complaints; write letters, make calls or deliver testimony concerning proposed legislation that affects people with disabilities; Document services provided according to policies and procedures; maintain clear, concise and accurate records in Center Database and record completed requests for Information and/or Referral; Assist in evaluation of consumers for program eligibility; Assist in the completion/development of consumer assessments and/or Care Plans; Review client care plans; Recruit and assist in maintaining a roster of potential personal attendants; Understand various benefit systems such as Vocational Rehabilitation, Medicare, Medicaid, Division of Family Services, Social Security and others. Be able to assist consumers in navigating these systems; Assist in planning programs and training relevant to CDS and beneficial to recipients; Make home visits to consumers homes; Travel to conferences or workshops, both in and out of state; Ensure safety and welfare of clients; Ensure prompt initiation of authorized services to new consumers; Notify clients of Client Rights; Ensure clients may present grievances and follow policies in regards to grievances; Report all instances of potential abuse, neglect, exploitation of a consumer, or any combination of these to the division’s Elder Abuse Hotline; Facilitate orientation and personal care training for personal care workers; Communicate with DHSS area case manager; Work occasional evenings or weekend days for special events or activities; Attend regular staff meetings as set by your supervisor; Understand and adhere to the Independent Living Philosophy. A Personal Attendant is an individual who is hired, trained, and supervised by a person with a disability (the consumer) to help with routines of daily living and other tasks they are unable to perform alone. A Personal Attendant can enable a person with a disability to reach a level of independence based on personal choices. Without the help of a personal attendant, many people with disabilities would be forced to live in a nursing home or other institution and would not be able to live in their own home.
